The following forms are available for download. You need the Adobe Acrobat Reader or Pro version 6 or later software to view, fill, and print the forms. NOTE: Most of these forms can be filled online, but require Version 6 of the Acrobat software and JavaScript to be enabled. If you have any problems with these forms, please notify the Board. NWASL Registration Form

This form must be filled by ALL players and submitted along with a filled and signed NWASL Conduct Form at the time of registration.

NWASL Conduct Form

This form must be filled and signed by ALL players along with their NWASL Registration Form.

NWASL Referee Evaluation Form

This form can be filled out by any referee, player, coach, or spectator to note the game performance of referees.

NWASL Team Roster Form

This form must be filled out by the team organizer and must include all the listed information for all team players.

Sponsor Agreement Form

This form must be filled out by any individual or organization who wishes to sponsor the Northwest Adult Soccer League through a donation. Sponsors are listed in the Sponsors section.

Misconduct Report

Referees must fill out and submit this form if any Cautions or Sendoffs occur in a game. Please submit the filled form and player card(s) to the Referee Administrator or the assigned representative within 24 hours of the game.

NWASL Player Feedback Form

Use this form to provide feedback to the Board. This feedback may include, but is not limited to, reports of player behavior, problems or questions about bracketing, registration, or other administrative functions, or just plain old kudos to the league, players, or referees!
